What actually happens during an ultrasound-guided injection

Most people picture an injection as a quick jab and hope. An ultrasound-guided injection is a different thing: the target is seen on screen, the needle is watched onto it, and the medicine goes exactly where the problem is.

Before the injection

The appointment starts the same way every assessment does – a history, a hands-on examination, and a diagnostic scan of the area. The scan is not a formality; it is what decides whether an injection is the right option at all, and if so, which one.

During the injection

The skin is cleaned and, where helpful, numbed with local anaesthetic. The probe sits over the target while the needle is introduced alongside it. Because the needle tip is visible throughout, most patients describe pressure rather than pain, and there are fewer attempts than with a landmark injection.

Afterwards

You leave with a written plan: what to expect over the next 48 hours, when to restart activity, and when a follow-up is useful. Responses vary between people and between structures; that is discussed honestly before anything is done, not after.

What this article is not

It is not a promise of outcome. Corticosteroid injections are prescription-only medicines and are only ever discussed in consultation; PRP and hyaluronic acid have mixed evidence and suit some patients and not others. The right answer is sometimes no injection at all.
